Case Management is a collaborative, solution-oriented service that helps patients access care, resources, and systems that support their mental and physical health.
At MH², case managers function as bridges between clinical treatment plans and daily life obstacles, providing practical assistance and ongoing communication.

Care Coordination

Resource Access

Behavioral & Practical Support

  • Strategies to reduce logistical barriers

Crisis & Stabilization Planning
