How the Program Works

Foundations for Recovery begins with a four-day Advanced Assessment that gives our team a deep understanding of your mental, metabolic, and physical health. Using this information, we create a personalized, three-month treatment plan tailored to your goals, symptoms, and lifestyle. Throughout the program, you’ll participate in a structured schedule that may include psychiatry, individual therapy, group therapy, metabolic and nutritional support, health coaching, and guided community outings. Your team meets regularly to review your progress, adjust your plan, and ensure every part of your care stays aligned with your needs. By the end of the program, you’ll leave with a clearer understanding of what’s driving your health challenges and a sustainable roadmap for long-term healing.

Therapeutic & Skills Groups

Flexible, Evolving Structure Through Case Management
Case management plays a central role in shaping a treatment plan that reflects each person’s unique goals, daily routines, and real-world challenges. Your case manager works closely with you to translate the treatment plan into practical steps, offering hands-on support with implementing new skills, building structure, and navigating day-to-day needs. This may include a tailored blend of in-home visits, virtual sessions, office appointments, groups, and community-based support—adjusted over time as your needs and readiness evolve.

Getting Started

The first month of Foundations for Recovery centers on your Advanced Assessment and initial orientation. During this time, you’ll meet with your psychiatrist, therapist, dietitian, and health coach for a thorough evaluation of your mental and metabolic health, nutrition, fitness, and lifestyle. This process helps your care team understand your unique needs, strengths, and goals. At the end of the month, your team will review all findings with you in a Treatment Recommendation Meeting (TRM), providing a personalized plan to guide your ongoing care.

After your assessment, the program shifts into structured, ongoing support tailored to your plan. You’ll participate in regular therapy sessions, psychiatric care, nutrition counseling, and health coaching, all designed to build routines, strengthen skills, and support overall well-being. Your team continues to review your progress and adjust your plan as needed, ensuring your care remains aligned with your goals and helps you make steady, meaningful progress throughout the program.
