Essential Assessment

The Essential Assessment follows a streamlined version of the same process ideal for people with more mild to moderate health concerns, led by our psychiatric nurse practitioner, it focuses on the essential elements necessary to understand a patient’s mental and metabolic health. Patients complete pre-visit questionnaires and then attend in-person appointments with our psychiatric nurse practitioner, psychotherapist, and health coach. Essential biometrics, labs, and cognitive testing are included.

What to Expect During Your Assessment

During your Essential Assessment, you will meet individually with members of our multidisciplinary team for comprehensive evaluations of your mental health, medical history, psychosocial factors, substance use, nutrition, lifestyle, sleep, and treatment goals. Standard biometrics, laboratory testing, cognitive testing, and a physical examination help provide a deeper understanding of the biological, psychological, and lifestyle factors contributing to your symptoms.

Throughout the assessment, our clinical team works together to integrate findings across disciplines, ensuring you receive a comprehensive understanding of your mental and metabolic health along with personalized treatment recommendations.

Your Personalized Treatment Plan

After your assessment is complete, our multidisciplinary team meets to integrate findings from every evaluation into a cohesive diagnostic formulation and personalized treatment plan. During your Treatment Recommendation Meeting, we’ll review the key findings, discuss the factors contributing to your symptoms, answer your questions, and provide clear short- and long-term recommendations to guide your care.
