Core Assessment
The Core Assessment utilizes a focused, multidisciplinary process centered on the essential components needed to understand a patient’s mental and metabolic health. Patients complete pre-visit questionnaires and participate in in-person appointments with psychiatry, therapy, and health coaching, along with core biometrics, laboratory testing, and clinical evaluations. The care team collaborates to develop integrated recommendations tailored to the patient’s individual needs.
It provides a comprehensive understanding of the patient’s overall health and a personalized report outlining next steps, while remaining less intensive than our Advanced Assessment.
What to Expect During Your Assessment
During your assessment, you will meet with members of our clinical team for comprehensive evaluations covering your mental health history, psychosocial factors, nutrition, lifestyle, and treatment goals. Standard biometrics, laboratory testing, and core clinical evaluations are included.
The Core Assessment provides a thorough diagnostic foundation while remaining less intensive than our Advanced Assessment.
Your care team works together to integrate findings across disciplines, ensuring you receive a cohesive picture of the metabolic, psychological, and lifestyle factors influencing your mental health.
What’s Included
The Core Assessment includes:
- Pre-visit questionnaires
- Psychiatric evaluation by a psychiatrist and/or psychiatric nurse practitioner
- Psychosocial and substance-use assessment
- Basic nutrition and lifestyle evaluation
- Fitness and movement assessment
- Sleep and environmental evaluation
- Cognitive testing
- Physical exam
- Core lab work
- Comprehensive treatment report (30–40 pages)
- Multidisciplinary team treatment recommendation meeting
The Core Assessment provides all core components needed to design a whole-person treatment plan, without the specialty in-depth assessments included in the Advanced Assessment—such as dietitian intake, advanced lab work, DEXA scanning, and extended psychiatry and therapy appointments.
After your appointments are complete, our team meets to integrate all findings into a cohesive diagnostic picture. During your Treatment Recommendation Meeting, you’ll review key findings from each discipline, diagnostic impressions, your personalized treatment plan, and short- and long-term recommendations.
